Service Point Form & Information
Service Point Form (PDF) | Service Point Form Guidelines | Volunteer Guidelines | Examples of Acceptable ServiceIf you have any further questions or concerns, pertaining to Service Point Forms, please contact Shawna Ford.
Service point forms are available on our website, in new member welcome folders, in campus libraries, in the Honors Study Room, and from regional campus advisors/campus liaisons. Completed forms should be mailed to the Recording Secretary, submitted during general meetings, or dropped off in the Phi Theta Kappa Honor Study Room in the McCormick Library.
Points for the year will be tallied and results available during the first meeting of the spring semester. Members achieving 100 or more volunteer service points during the calendar year (January - December) will be recognized during the Alpha Nu Omega Chapter's annual Awards & Recognition Banquet held in April.
Members are responsible for keeping track of their service points. However, if there are questions or if a member wants to confirm his/her service points, members may contact the Recording Secretary. Questions regarding service activities (what qualifies and what doesn't) should be directed to Founding Chapter Advisor, Dean Thaddeus Sampson or any one of your Executive Board members.
